Being a Mote volunteer is a position of significant importance to Mote Marine Laboratory with associated responsibilities. Each volunteer is asked to agree to the standards we have set to assure that their intentions as a volunteer are in alliance with Mote's mission. Please check each box to acknowledge that you have read and agree to the standards listed below.

I realize that by agreeing to be a volunteer for Mote Marine Laboratory, I am making a commitment to a position that requires both responsibility and accountability. I agree to report for my assigned shifts and to meetings on a punctual and consistent basis and to perform to the best of my abilities. I will notify my supervisor of any absences with as much lead time as possible. I understand that my performance as a Mote Marine Laboratory Volunteer is subject to review by appropriate Mote staff. I realize that unsatisfactory performance may result in change of assignment or termination of my participation in Mote's volunteer program.

I do hereby assume the risk of all dangerous conditions in and about the property of MML or its event sites; waive any and all specific notice of the existence of dangerous conditions; and indemnify and hold harmless Mote Marine Laboratory, its trustees, officers, agents, or employees from any and all liability, actions, causes of actions, debts, claims, demands or other liability of any kind or nature whatsoever which may arise, whether caused by ordinary negligence or otherwise. I intend that this agreement shall serve as a release, waiver, and assumption of risk and shall bind any person or entity making a claim based upon my rights or on my behalf, including, but not limited to, my heirs, executor, assigns, or members of my family. If any portions of this release are found invalid, the balance shall remain in full force and effect.

I hereby understand and acknowledge that, unless otherwise defined by applicable law, any volunteer relationship with Mote Marine Laboratory is of an "at will" nature, which means that the volunteer may resign at any time and Mote Marine Laboratory may discontinue the relationship with the volunteer at any time with or without cause. It is further understood that this "at will" relationship may not be changed by any written document or by conduct unless such change is specifically acknowledged in writing by the Chief Executive Officer of Mote Marine Laboratory.

I certify that answers given herein are true and complete to the best of my knowledge. I authorize investigation of all statements contained in this application as may be necessary.
